Aftercare Community Based Support Programs
Youth accomplish a great deal while in residential programs, but research shows that without support much of that success will be undone. Rebound prides itself on staying connected with the amazing young people who come through our residential programs, and being available as a place they can return to for support. The Aftercare program formalizes this support in hopes of helping our youth retain gains made while in placement and achieve their full potential!
Aftercare Transitional Support
After Care Transitional Support Program “ATS” is a two-year, drug and alcohol-free, transitional support program.

Target Population
- ALL youth who have participated in Rebound, Inc. residential programs are eligible to participate in aftercare services REGARDLESS of whether they exit the program "successfully" or otherwise.
- Ramsey County youth transitioning to community after out of home placement.

Overview
- Youth will spend 25-30 hours each week working towards program goals. These hours may include work, school, weekly groups, therapy, treatment, meetings with rebound Staff, and other activities as approved by the Youth's Case Manager. Youth and their Case Manager will discuss program expectations, set goals and plan for services that are helpful to meet those goals. They will make the final decision regarding those services, and will attend Meetings with their Case Manager at least once a week, as well as attend other scheduled meetings and workshops.
- Participation in the ATS will not exceed twenty-four (24) months. Rebound may dismiss youth from the program and stop providing housing subsidy for violating the rules. Youth can leave the program at any time by giving staff at least 30-days written notice. If they choose to leave the program, they will be responsible for the full payment of their monthly rent, due to UHW.
- Rebound, Inc. partners with Urban Homeworks (UHW) to place ATS Program participants in apartments. Participants then hold a lease directly with UHW and are subject to all rights and responsibilities as a tenant. Because of this, participation in the ATS program can be terminated without the requirement that youth vacate their apartment. However, if they are terminated from the ATS program, their rent subsidy will stop.
- Rent subsidies are based on unit size and youth's income. Youth will not pay more than 30% of their monthly income, up to 50% of total unit rent. A rent subsidy will pay the remaining balance of their rent, provided that they remain in the program. Youth's portion of the rent is due by the 1st of each month TO REBOUND.
- Rebound will pay the youth's rent by the 1st of each month. Their rent is late if it is not paid by the 5th of the month. If rent is paid after the 1st of the month, a $25 late fee is due to REBOUND AT THE TIME OF PAYMENT. If rent is not paid by the 15th of the month youth may be subject to program termination.
Community Based Support Program (CBSP)
Service Description
Rebound, Inc. will provide support to youth who are in placement or are at risk for out of home/correctional placement, as a means to retain them successfully in the community. Program components will include six (6) months of:

- Intensive outreach case management, 2 weekly 1:1 meeting.
- Transportation to school meetings & job interviews
- Monthly Community Reintegration Outings
- Quarterly opportunities to expand their horizons & explore their passions through camping trips, collegetours, participation in model assembly, etc.
- Assistance with probation case plan follow through
- Weekly Individual Therapy as needed
- Family Therapy as needed
- Psychiatry as needed
Target Population
The CBSP Program can serve up to 15 youth at a time, and will aim to serve 30 youth per year. Eligible youth will meet the following criteria:
