clinical Case manager position
Rebound, Inc. provides residential group home services to up to 6 youth between the ages of 14 and 18+. The goal of Rebound’s Residential Program is to provide these young people with the holistic programming and services that they need to live safely and successfully within their communities.
Under the supervision of a licensed social worker, the Clinical Case Manager is responsible for the development and implementation of a plan to achieve this goal. The Clinical Case Manager conducts resident intakes through which they identify resident safety needs, and create a safety plan. The Clinical Case Manager implements, monitors and evaluates the safety plan. Additionally, the Clinical Case Manager plans and facilitates Psychosocial groups and supports resident participation in recreational activities. |

- Master’s Degree in Behavioral Sciences OR Enrolled in an accredited Master’s program AND experience with youth/young adults Required license(s):

Duties & Responsibilities:
- Effectively engage individuals and families to assess risks and safety.
- Collaborate with agencies and residents to determine eligibility, need, and level of service for identified residents.
- Construct and implement an Individual Safety Plan.
- Coordinate and provide direct services and advocacy to residents and families.
- Facilitate access to and integrate community resources on behalf of the resident.
- Evaluate, document, and monitor progress toward goals and objectives.
- Maintain and apply knowledge of relevant policies, procedures, and statutes and professional practice.
- Complete agency, state, and federal required documentation as related to resident activity according to mandated timelines.
- Provide immediate response in crisis situations. Maintain safety for residents and community.
- Develop and implement a safety plan for residents to maximize their safety and minimize community risk.
- Facilitate Psychosocial groups for residents.
- Supervising and supporting resident participation in recreational opportunities.
- Construct and implement an Individual Safety Plan
- Assist with transition planning to promote or ensure resident success after discharge.
Knowledge, Skills, & Abilities:
- Knowledge of human development, family dynamics, and systems, including crisis management and cultural competency.
- Knowledge of mental health interventions and best practices
- Knowledge of counseling, skill-building, therapy, advocacy, case management, crisis intervention, or narrative therapy services.
- Knowledge of community resources and funding sources.
- Knowledge of medical diagnoses, medications, and basic medical terminology, as well as medical insurance processes and procedures.
- Knowledge of statutes, rules, and policies pertaining to social work and the ability to research the same.
- Interviewing, negotiation, and listening skills.
- Ability to communicate effectively both orally and in writing.
- Knowledge of evidence-based practices including assessment, case planning, and cognitive interventions.
- Knowledge of criminal behavior and/or juvenile delinquency.
- Knowledge of basic psychology.
- Knowledge of group dynamics.
- Knowledge of the criminal justice system in Minnesota; working knowledge of the functions of its component parts.
- Ability to make decisions under stress, while abiding by Departmental and State policies and guidelines.
- Considerable ability to work effectively with the public, department and County associates in a professional manner.
- Ability to work in a team to accomplish tasks, plan and resolve problems.
- Ability to communicate effectively both orally and in writing.
- Ability to apply departmental policies and procedures to assigned duties.

Work Environment:
- Clinical Case Managers are expected to work on-site at Rebound properties as well as in the community.
- The Clinical Case Manager will also have on-call shifts each month.

Compensation:
- $58,000 - $66,000 Annually; Exempt position. DOQ

Senior Youth Engagement Specialist
Rebound’s Residential programs offer group home and shelter services to up to 12 young people between the ages of 14 and 21 each day. Services are delivered at two group homes: Jelani House in St. Paul and Naima House in Minneapolis. The Lead Youth Engagement Specialist will help Rebound deliver holistic programming and services 24 hours a day, every day of the year. Under the supervision of a mental health professional, the Lead Youth Engagement Specialist promotes a safe and welcoming environment for all residents and their families.
Minimum Qualifications:: Bachelor’s Degree in Behavioral Sciences AND knowledge of mental health services AND one year of clinical experience; OR Enrolled in an accredited Master’s program AND trained in working with youth in a clinical setting OR Master’s Degree in Behavioral Sciences Preferred Qualifications: Master’s Degree in Behavioral Sciences plus 2 years of clinical experience |

- Regular attendance according to the position’s management-approved work schedule is required for all positions.
- Supervise residents in the house and during recreational activities in the community.
- Intake support for case manager(s) including administering and scoring screenings for new residents within prescribed timelines.
- Implement Individual Safety & Behavioral Modification Plans as co-developed with Clinical case manager.
- Evaluate, document, and monitor progress toward goals and objectives.
- De-escalation and crisis support as needed to support residents’ case plan goals.
- Role model positive interactions for residents and colleagues.
- Co-facilitate psychosocial groups.
- Work collaboratively with the youth, case manager, staff, and administrators to identify, support, and document progress toward resident behavior modification goals on progress trackers, in self-reflection files, and through youth modules.
- Ensure clear, accurate, and timely documentation by self and co-workers.
- Provide immediate response in crisis situations. Maintain safety for residents and community.
- Ensure compliance with house schedule, rules, and expectations.
- Other tasks as assigned to support Rebound’s programming/mission.

About Ujima: Ujima is a trauma-based program dedicated to working with families in our community. Our goal is to build coping skills and resources to prevent relapse and re-exposure to trauma. We utilize the S.E.L.F Trauma-Informed Psychoeducation Group Curriculum by Sandra L. Bloom, MD. Facilitators will receive comprehensive training on the curriculum. Better Together - Mentors
Rebound, Inc. is currently seeking volunteers to serve as Mentors in the Better Together Youth Mentorship Program. Mentors commit to:
- Undergo screening to ensure they are fit to work with youth,
- Complete 4 hours of orientation training,
- Complete 2 hours of supplemental training monthly,
- Apply learnings from training to work with youth,
- Reach out to mentorship program coordinator for support as needed.
- Meet with mentee at least ONCE per week for ONE YEAR
- Attend quarterly matched pair activities
- Submit monthly “Match check in sheets”
